Please see the attached document for the 26-27 AIA Athletic Physical Form. There are multiple versions out there of this, but the attached version here covers all of the necessary forms we need to clear your athlete. The forms are as follows:

Page 1-3 - General Info and Athlete History

Page 4 - Suicide/Crisis Helpline (not needed in submission)

Page 5 - Family History

Page 8 - Examination Form. Must be completed by a medical provider.

Page 9 - Acknowledgement of Brain Injury/Concussion Form

Page 10 - Consent to Treat Form

Physicals must be completed on or after 3/1/26 to be acceptable for the 26-27 school year. Please be on the lookout for our school-wide physical day that will allow you to get your physical completed with the money going back to our athletics department. This will be taking place on Saturday, 5/9/26 from 8:00-12:00pm at OrthoArizona - South Gilbert. Signup link will be posted here soon!
